Despite our best efforts there will be occasions when things go wrong. One of the most important things we can do is to ensure that these events are seen as an opportunity for learning and improving systems to reduce the risk of similar problems in future.

Clinical audit
We do not have to wait for things to go wrong, to put measures in place to improve the quality of care. Clinical audit provides a method for involving the team in improving the quality of care and these references may give you some ideas on where to start .
